This describes the process followed by the Community Mental Health Teams when supporting those with mental health needs.
It is built around four key aspects:
- Assessment – identifying and assessing the health and social care needs of the person
- Care plan – a plan for meeting the assessed needs
- Care Co-ordinator – appointing a key worker to monitor the implementation of the care plan and to keep in close contact with the person
- Review – regular review and, where necessary, revision and changes to the care plan
