Catch 22 supports care leavers with the transition from the care system to adulthood and independent living, including accessing suitable and secure accommodation. Kent 16+ operates a supported lodging scheme for young people whereby they can live within a family-type environment whilst acquiring the independent living skills and emotional resilience necessary to enable them to live independently. The carers within this scheme are approved and receive comprehensive training and on-going support and supervision from the dedicated supported accommodation officers within the service.

Catch 22 services is committed to ensuring young people have the best possible chance to succeed in life, with support including:

• a plan to keep track of the important things (a Pathway Plan)

Centra Support provides support for people who experience mental illness, older people, domestic abuse, young people, homelessness and learning disabilities with their accommodation needs. Centra Support projects are accommodation based. Centra Support provide a person centred approach, supporting people to achieve their aspirations, working closely with other agencies. There are five mental health schemes in Centra Kent. Four are accommodation based, with self contained flats and communal areas.

The Horizons projects, Archer House and George Culmer Court, are jointly managed with KMPT. People have more intensive support needs around both housing and mental health and may remain in their accommodation for some years. Referrals need to have a diagnosis of a mental illness and to be already receiving support from a CMHT.

Chaucer House and Lower Northdown Avenue are funded to support people with low or medium needs. The aim is for people to be living independently in the community within two years.

There is also another scheme, Release Planning and Re-enablement. (0238 1848729). Centra Support work in partnership with Oxleas NHS Trust with clients in Kent prisons who experience mental health and/or substance misuse issues and who are homeless. Centra Support help identify accommodation and provide a floating support service for up to 3 months upon release from prison.

The Richmond Fellowship provide a range of housing related support to enable individuals with mental health problems to develop and maintain independence in the community. Their services include:

The service promotes social inclusion for people with mental health difficulties by supporting them to access group or 1:1 support depending on what support they require. The Thanet Way supports individuals who have or are recovering from mental illness to access and sustain meaningful activity. This could be paid work, voluntary work, training or educational opportunities. The support is on a one-to-one basis and works toward building an action plan for every individual, to set goals/targets whilst working to time scales and with identified responsibilities. The Thanet Way service works with referrals from the Thanet Community Mental Health Team.

Windmill Community Gardens is a community garden project in Thanet. The Windmill Community Garden has community plots, wildlife habitats and Organic Food growing plot on the Dane Valley Allotments, Margate.
The Windmill Community Garden project has a range of opportunities for people to come together to make a difference for themselves and help to improve their physical and mental health, social isolation, wellbeing and community cohesion through educational and environmental activities. Activities in the Windmill Community Garden include:

• A Market Garden: Growing a variety of Organic vegetables and run a weekly vegetable bag scheme with a mixture of the produce we grow and also from local farmers.
• Garden Club for people to drop into, you do not have to be an expert gardener, just come along and share any skills not just gardening.
• Cabbage Patch parent and toddler group for families with a child aged 5 or under on Thursdays. Here families are encouraged and supported to join in with healthy, seasonal cooking, wildlife, art & craft activities, explore the gardens, pond dipping, herb walks, socialize and support one another. Cost £1 for first child 50p for any additional children.
• Wellbeing Group supporting people with Mental Health needs where participants can be referred by their GP or support worker to attend. People can again join in with various activities including cooking, harvesting, planting, craft, art, socializing and so much more.
• On Saturdays The Funtastic Kids Saturday Outdoor Club runs for 8-13 year olds where children & young people can play, explore, cook & eat, pond dip, grow, harvest, craft, water plants, dig, and play.
