Community Mental Health and Wellbeing

A new Community Mental Health and Wellbeing Service for Kent has been commissioned. The new service will start from the 1 April 2016 and aims to keep people well and support people with mental health needs on their recovery journey. The new Community Mental Health and Wellbeing Service will provide person centred support that champions mental wellbeing within local communities, builds resilience and support people to live well.

The new Community Mental Health and Wellbeing Service forms a key part of an integrated care pathway bringing community services health and social care together. Commissioners have ensured that the service will focus on the outcomes that matter most to people such as gaining employment, finding a suitable home and access activities of interest including sports, leisure, art and culture.

The service will be delivered in the following four areas by the following Strategic Partners:

1. Dartford, Gravesham, Swanley and Swale CCG areas - Porchlight

2. West Kent CCG area - Shaw Trust

3. Ashford and Canterbury Coastal CCG areas - Shaw Trust

4. Thanet and South Kent Coast CCG areas - Porchlight
